Job Summary

Aparaprofessional person who is specifically trained, competent and performs assigned functions of personal care to the individual in their residence under the direction, instruction and supervision of the DD CEO/Supervisor.

Qualifications:

  1. Have a strong desire to enhance the quality of life of those served.
  2. Ability to carry out directions, read and write.
  3. Maturity and ability to deal effectively with the demands of the job

Responsibilities:

  1. Understand and adheres to established Agency policies and procedures.
  2. Performs personal care and bath as ordered.
  3. Completes appropriate documentation in a timely manner as per agency policy.
  4. Reports changes in the individual's condition and needs to the DD CEO/Supervisor.
  5. Performs household services essential to health care in the home as assigned.
  6. Ambulates and exercises the individuals as assigned.
  7. Performs simple procedures as an extension of the therapy services, e.g. range of motion (ROM) exercises as assigned.
  8. Assists with medications that are ordinarily self-administered as assigned.
  9. Attends in-service and continuing education programs as scheduled and necessary.
  10. Attends specific individual meetings as scheduled.

Working Environment:

Works indoors in individual homes, and travels to/from individual homes. Also, facilitates neighborhood/community outings as directed by the individual served.

Lifting Requirements:

Ability to perform the following tasks if necessary:

  • Ability to participate in physical activity
  • Ability to work for extended period of time while standing and being involved in physical activity
  • Heavy lifting
  • Ability to do extensive bending, lifting and standing on a regular basis.

Events that must be reported immediately, but no later than four hours after discovery of the incident, to the local county board include:

  • Accidental or suspicious death;
  • Exploitation;
  • Misappropriation;
  • Neglect;
  • Peer-to-peer acts;
  • Physical abuse;
  • Sexual abuse;
  • Verbal abuse; and
  • When any staff member has received an inquiry from the media regarding a major unusual incident.

*Even if any of these events occur while you are not providing service to the individual but hear about it, you must please report it immediately.

Behaviors that are not allowed when caring for the individual:

  • Discussion of politics, religion or personal affairs
  • Consuming individual's food or drink without permission
  • Use of individual's vehicle or property without permission
  • Bring friends, family or pets to the individual's home
  • Taking the individual to your home or taking individual to another individual's home, unless the activity is authorized in the individual’s service plan
  • Consuming alcohol or drugs that may impair ability to work
  • Being involved in any money transactions other than those directed by the individual
  • Leaving an individual's home for unrelated reasons
  • Engaging in activities that distract from service
  • Sharing an individual's information with someone
  • Talking/texting on your personal phone unless speaking with a Halo administrator
  • Smoking around the client
  • Activities that create a ‘Conflict of Interest’ and which may limit decision-making for the individual
