PRE-EMPLOYMENT STATEMENT

hereby affirm that all information provided in my application for employment with Steps 2 Better Living, Inc. is true and complete to the best of my knowledge. I understand that any false statements, misrepresentations, or omissions may result in disqualification from employment consideration or, if employed, disciplinary action up to and including termination.

I acknowledge that employment with Steps 2 Better Living, Inc. is contingent upon the following conditions:

  1. Background Check & Verification: I authorize Steps 2 Better Living, Inc. to conduct a background check, including but not limited to criminal history, previous employment verification, educational background, and professional references. I understand that my employment is contingent upon the results of these verifications.
  2. BCI & FBI Background Checks (Ohio Residents): If I reside in the state of Ohio, I agree to submit to a Bureau of Criminal Investigation (BCI) background check as a condition of employment. If I have not resided in Ohio for the past five years, I also agree to submit to an FBI background check in addition to the BCI check.
  3. Drug Testing: I understand that I may be required to undergo drug and alcohol testing as part of the hiring process and, if employed, as a condition of continued employment.
  4. Employment At-Will: I understand that employment with Steps 2 Better Living, Inc. is at-will, meaning that either the employer or I may terminate the employment relationship at any time, with or without cause or notice, except as otherwise provided by law.
  5. Compliance with Policies & Procedures: If employed, I agree to abide by all policies, procedures, and rules established by Steps 2 Better Living, Inc., including but not limited to workplace conduct, confidentiality, and professional standards.
  6. Confidentiality Agreement: I understand that during my employment, I may have access to sensitive and confidential information regarding clients, employees, and the organization. I agree to maintain strict confidentiality and to follow all applicable laws and policies regarding the protection of such information.
  7. Legal Right to Work: I affirm that I am legally authorized to work in the United States and will provide documentation verifying my identity and work authorization as required by law.
  8. Physical & Mental Fitness: I acknowledge that my employment may require certain physical and mental capabilities necessary to perform essential job functions. I agree to undergo any required health screenings or evaluations that may be necessary to assess my ability to perform my job duties.
  9. Acknowledgment of No Guarantee of Employment: I understand that the submission of this application does not guarantee employment and that no representative of Steps 2 Better Living, Inc. has the authority to enter into any employment contract unless stated in writing and signed by an authorized executive of the company.

As part of the hiring process for the position of Direct Support Professional (DSP), Steps 2 Better Living Inc. requires a background check to ensure the safety and well-being of the individuals we serve. This background check may include, but is not limited to, the following:

  • Criminal history
  • Employment verification
  • Education verification
  • Reference checks
  • Driving record (if applicable)
  • Drug screening (if applicable)

By signing this document, you authorize Steps 2 Better Living Inc. to conduct a thorough background check as part of the hiring process.

Consent and Authorization:

hereby authorize Steps 2 Better Living Inc. and its designated agents to conduct a background check for employment purposes. I understand that this background check may include information regarding my criminal history, employment history, education, and other relevant records.

I release Steps 2 Better Living Inc., its agents, and any third-party providers from any liability or claims arising from the use of the information obtained through this background check.

I also understand that the results of this background check may affect my eligibility for employment with Steps 2 Better Living Inc. If I have any questions or concerns about this process, I will contact the hiring manager or human resources representative.
